Hospice Care Group offers end of life care, symptom management, and primary care services. They specialize in supporting individuals and families who are facing the challenges confronted when dealing with terminal and chronic illnesses. Their goal is to ensure that their patients are experiencing a final transition on their own specified terms — in comfort and with dignity. In their care, the main objective is always to maximize patients’ comfort and quality of life.

Problems We Solved

Hospice Care Group (HCG) reached out to CopperHill to specifically work on scalability issues, Inovalon integration for benefits verification, Homecare Homebase integration, and duplicate effort reduction. Their in-depth assessment process and increased referrals from differing sources caused multiple challenges.

Some specific problems we solved for Hospice Care Group were:

  • Need for better referral tracking and central repository for referrals
  • Need for streamlined verification of insurance benefits
  • Need to formalize patient onboarding to save time on intake
  • Need to consolidate transactional and clinical systems, and share information across those systems

Scalability Issues: HCG had a legacy Salesforce instance and needed to upgrade it to support day-to-day operations. 

Inovalon Integration: Custom integration built from the ground up with real-time data transfer. Used the Inovalon ONE Platform to record and verify insurance benefits inside the CRM and EHR for HCG.

Homecare Homebase Integration: Hospice Care Group discovered that they were only able to read data from Homecare Homebase (HCHB). This was causing administrative issues for HCG and bidirectional synchronization was critical for the success of the project. With the help of CopperHill they were able to get access to HCHB’s API, which solved their synchronization issues.

Reduce Duplicate Efforts:  The goal was to automate and track as much as possible to reduce swivel-chair at Hospice Care Group.

Solution Design

CopperHill is an integration expert that uses Salesforce to support Hospice Care Group’s referral management and patient onboarding process. This creates uniformity within the organization and across different systems. 

Our integration work with HCG revolved around multiple accelerators such as NextGen, Homecare Homebase, Allscripts, and Inovalon. We translated data from HL7 and FHIR standards to make it shareable across these EHRs and Salesforce. 




Salesforce is the single source of truth for patient management–the patient will originate in Salesforce, with patient details and insurance verification. This information is then pushed to the EMR for providers to access. The final result is Hospice Care Group has the ability to simplify, automate, and track the full patient cycle from outreach to intake to care management-with Salesforce as the source of truth.
